Low Dist. Amazing Spider-Man #688, written by Dan Slott, was previewed as "Spider-Man is stuck between two monsters when Morbius the Living Vampire attempts to cure the Lizard and restore Curt Connors".
The comic was published with both a direct edition (Diamond code: APR120615) and a newstand edition back in 2012 both with the same Stefano Caselli cover (cover A), along with two variant covers:

PGX and CGC categorize this simply as "Variant Edition" whereas CBCS have it as "Campbell Retailer Incentive" and note a number of details around the content:
In July 2016, CGC had 67 recorded gradings of the "Variant Edition" with 22 in the highest grade of 9.8 (note: neither CBCS nor PGX had a published census of graded comics at that time).
